JANUARY TOPIC – GOALS
Prepare a five-minute story about aiming high. Buckling down and getting to work or rain-checking your ambitions until the new year. Learning to bake sourdough, finishing that script, perfecting the three-ball juggle…or falling short despite your blood, sweat, and tears. Digging in or checking out. The sky’s the limit – dream big!
MOTH COVID-19 Policy
With the health and safety of our local storytelling community being a priority of the organizers, proof of vaccination will now be required for admittance to the Louisville Moth StorySLAM. A rise in cases and emergence of new variants makes negative tests no longer sufficient for entry. Thank you for supporting the Louisville Moth and helping protect your fellow audience members!
Masks will be required for all staff and encouraged for all patrons.
Your vaccination card can be a physical or digital copy and must match your ID. Please direct any questions to firstname.lastname@example.org.
By entering the venue, you assume all the risks associated with COVID-19. Furthermore you release Jump In, LLC and Production Simple, LLC from all liability associated with COVID-19.
COVID WAIVER FOR TICKETBUYERS:
While The Moth and Event Venue are taking measures to enhance the safety of our attendees, crew, performers, and staff, due to the contagious nature of Coronavirus/COVID-19 (“COVID-19”), we require all attendees to read and accept the COVID-19 Liability Waiver below:
By purchasing tickets and entering the Event, I agree to comply with all policies, guidelines, and preventative measures implemented by The Moth and/or the Event Venue designed to reduce the spread of Coronavirus/COVID-19 (“COVID-19”), as well as applicable federal, state, and local government rules, regulations, and recommendations.
I acknowledge: Despite The Moth and the Event Venue’s efforts to enact health and safety measures, entering the Event still may pose a risk to my safety and presents an increased risk of exposure to and infection with COVID-19.
● I accept the risk of being exposed to, contracting, and/or spreading COVID-19 along with all consequences resulting from exposure or infection, including but not limited to physical injury, illness, medical treatment, property damage, temporary or permanent disability, death, and economic or financial loss that may arise from, or in connection to, The Moth’s Event. I understand that this waiver means I give up my right (and the right of my heirs and/or personal representatives) to bring any and all claims, including, but not limited to claims of negligence against The Moth and/or Event Venue.
● I agree to comply with the rules and regulations which The Moth and/or Event Venue may establish at the time to protect all involved with the Event, including but not limited to: wearing a face covering over nose and mouth, and a health screening process, which may include but not be limited to a temperature check and my certification that at the time of entry I have not been diagnosed with, had close contact with someone who was diagnosed with, or exhibited known symptoms of, COVID-19 in the last 14 days.
By purchasing this ticket, I acknowledge that myself and any additional members of my party will provide valid proof or documentation that I / we are: either fully vaccinated: defined as being 2 or more weeks after the final dose (first for Johnson & Johnson, second for Pfizer and Moderna); or have had a negative COVID-19 test result within the past three days of event date.
